Telephone Self-Service
Telephone Self-Service is an automated option that makes it easy for military retirees to have their 1099-R mailed to the mailing address DFAS has on record.
This option can be used 24 hours a day, 7 days a week. There is no need to speak with a representative, wait on hold, or even use a computer.
Use the following instructions to get your 1099-R through Telephone Self-Service:
- Call 1-800-321-1080 or 317-212-0551
- Select option “1” for Self-Serve
- Select option “1” to request copies of your tax documents
- Select option "1” 1099-R
- Enter Social Security Number when prompted
Your 1099-R should be in the mail within 7-10 business days to the address we have on record.
Please note that 1099-R reissues requested through the telephone self-service option cannot be mailed prior to February 12, 2024.
