Marines: Complete claims package (by dependent type)

Your secondary dependent claim package should include the following documents and forms (based on dependent relationship). Be sute to include all the items and that forms are filled out completely with all required signatures. Be sure the forms are legible to minimize any delays in processing.

PARENTS, PARENTS-IN-LAW & STEPPARENTS

DD Form 137-3 - Dependency Statement - Parent (Must be completed by parent. Notary seal/signature is required.)
NAVMC Form 10922 - Dependency Application (Must be signed my you and the attesting officer)
Verification of dependent's income
Proof of support when dependent does not reside with you

 

IN LOCO PARENTIS (In place of parent)

DD Form 137-3  Dependency statement - parent (Must be completed by the adult who has physical guardianship of the child. A notary seal/signature is required on this document
NAVMC Form 10922 - Dependency Application (Must be signed by you and the attesting officer)
In loco parentis affadavit - This document is to be completed by the claimed dependent, not by you or your command. Proof of the claimed dependent's income and your monthly contribution must be provided.
NOTE: In loco parentis dependents are NOT eligible for a USIP card.

 

LEGAL CUSTODY WARDS
FORMS/DOCUMENTS
DD Form 137-7 Dependency statement - ward of a court (Must be completed by the adult who has physical guardianship of the child. A notary seal/signature is required on this document
NAVMC Form 10922 - Dependency Application (Must be signed by you and the attesting officer)
Court document granting guardianship of the child to the member – power of attorney or signed statements from the parents granting custody of the child to the member are not acceptable documents
Child's birth certificate – only required if the child's birth date is not stated in the court document
Verification of Income and Proof of Support when dependent does not reside with you due to military necessity or incapacitation.

Statement from member stating that the child will reside in the member’s household for 12 months or longer

Proof of the dependent's income (if applicable)

 

INCAPACITATED CHILD (21 YEARS OR OVER)
FORMS/DOCUMENTS
DD Form 137-5 - Dependency Statement - incapacitated child over 21 (A notary seal/signature is required on this document)
NAVMC Form 10922 - Dependency Application (Must be signed by you and the attesting officer)
Medical sufficiency letter from a uniformed services medical provider or civilian physician. Use our example of a medical sufficiency letter to get started.

• A recent (within the last 4 months) medical or psychiatric evaluation and diagnosis
• A complete, detailed medical summary of illness, to include the date and child's age at onset of incapacity
• Current treatment being rendered and prognosis for recovery, as well as the ability to become self-supporting
• A DSM-III diagnosis in all cases of mental retardation

Verification of Income and Proof of Support

 

FULL TIME STUDENT (21 OR 22 YEARS OLD)
FORMS/DOCUMENTS
DD Form 137-6 Dependency statement - full-time student 21-22 years of age (A notary seal/signature is required on this document. Must include a letter from an institution of higher education showing full-time enrollment and anticipated date of graduation.)
NAVMC Form 10922 - Dependency Application (Must be signed by you and the attesting officer)
Verification of Income and Proof of Support


When complete, submit your dependency package to your command admin office.
