Your secondary dependent claim package should include the following documents and forms (based on dependent relationship). Be sure to include all the items and that forms are filled out completely with all required signatures. Be sure the forms are legible to minimize any delays in processing.
Make sure you send your secondary dependency claim to the correct office. Offices responsible for determining dependency based on the type of dependent are included below. Check out our Helpful Hints page for use in filling out the forms required for your secondary dependency claims package. NOTE: The forms listed in the tables below are linked to online official forms for your use.
Proof of Support: Proof of the member’s monthly contribution is required.
Please note: Parent, Parent-in-Law, Step Parent or Parent by Adoption and In Loco Parentis must explain change in their financial circumstances that makes them dependent on Navy member for more than half support.
Proof of support: Proof of the member’s monthly contribution in cases where the member is deployed.
Please Note: Member must show where he/she established a residency and the ward cohabitated with him/her prior to deploying.
