SUBMIT A PRIVACY ACT REQUEST

Only the individual to whom the record(s) pertain or their legal representative [with appropriate documentation] may submit a Privacy Act (PA) request. The information requested must be maintained within a System of Record in the custody of the Defense Finance and Accounting Service (DFAS) at the time of your submission.
 

Under FOIA/PA, federal agencies ARE NOT REQUIRED TO:
  • Answer questions,
  • Analyze data,
  • Conduct research,
  • Perform calculations or
  • Create/compile records in response to a request.

 The Freedom of Information Act/Privacy Act (FOIA/PA) Office is not a repository for documents maintained or released by DFAS. Requests received in this office are forwarded to the functional area that has the responsibility for the subject matter requested.
 
DFAS was established in 1991. We began processing DoD civilian payrolls in 1995 and the Department of Veterans’ Affairs in approximately 2006. Therefore, we do not have ANY records or information regarding individuals who were federal civilian employees prior to these years. Please direct your request to the National Archives and Records Administration.
 
Documents that may be requested includes pay records, debt records, tax forms, etc.

DFAS DOES NOT REQUIRE A SPECIFIC FORM
TO MAKE A PRIVACY REQUEST; HOWEVER, YOUR SUBMISSION MUST CONTAIN THE BELOW INFORMATION
 

Individual to Whom the Information Pertains
  • Name
  • Full Social Security number
  • Contact Information (phone number, email address)
  • Proof of Identify
    • Copy (front and back) of a valid, government issued photo ID
    • OR
    • A Notarized Statement that verifies your identity

Description of Requested Information

  • Be as specific and detailed as possible
  • Search period - Start Date (Month/Year) AND End Date (Month/Year) 
  • Form number (if known)
  • DO NOT use ‘any and/or all…information’, dates of birth, enlistment or separation, etc.; these are not a searchable parameters

Your Status for the Requested Period(s)

  • Federal Civilian Employee
  • Military Service Member

If you are/were a Federal Civilian Employee
  • Name of agency or agencies for which you works/worked 
  • Dates of Employment for each agency

If you are/were a Military Service Member

  • Branch of Service – Air Force, Army, Marine Corps, Navy, Space Force
  • Component – Active Duty, Reserve, National Guard, Military Academy
  • If you served in multiple branches/components, list each service period separately 

If you are requesting Army Reserve or Army National Guard information that is dated prior to 1976, we also require:

  • Name(s) of the Major Command(s) you were assigned to during the requested period(s)
  • Name(s) of the Unit(s) of Assignment for each command
  • City/State for each Unit of Assignment
  • Dates you served in each Unit
 
In lieu of a written/typed statement, military members and federal civilians may utilize the Request Pertaining to Military Records (SF-180) to request your personal records or authorize their release to a third party. A copy of your Photo ID or Notarized Statement, as detailed above, must accompany the signed SF-180.
