SUBPOENA AND COURT ORDER

INVESTIGATORS
Agents who require records for investigatory purposes (fraud, theft, etc.) please forward your request to the DFAS Internal Review, Criminal Investigations Directorate (dfas.dscc.hi.mbx.rfa@mail.mil).
 
FEDERAL EMPLOYEES (NON-INVESTIGATORS)
The DFAS Freedom of Information Act/Privacy Act (FOIA/PA) office does not require federal employees who are making a request, in their official capacity, to obtain a subpoena/court or coordinate with our office to obtain records. You may contact the DFAS mission area that has responsibility for the records directly. If you cannot locate a point of contact, we can provide one.

If you prefer to utilize the FOIA/PA office, your request must include:
 
  • Name and full social security number of the individual to whom the records pertain
  • Individual’s branch of service or agency name
  • Description of requested records
  • Search period: Start date (month/year) and End date (month/year)
  • Reason you are requesting the records
 
Requests submitted electronically must be from your official government email address.
If you submit via fax, the request must be on official letterhead and signed.
