AN INDIVIDUAL’S AGREEMENT [OR NON-OBJECTION] TO THE ISSUANCE OF A SUBPOENA DOES NOT CONSTITUTE CONSENT OR AUTHORIZATION TO RELEASE HIS/HER PERSONAL RECORDS OR INFORMATION |
The FOIA/Privacy Office does not require a specific form however, PROPER CONSENT/AUTHORIZATION to release personal records or information requires the following:
STATEMENT FROM THE INDIVIDUAL TO WHOM THE RECORDS PERTAIN:
- Name
- Full Social Security Number
- Contact Information – phone number, email address
- Specifically authorizes the Defense Finance and Accounting Service (DFAS) to release his/her personal records
- List records that are to be released
- Date
- Signature
- Copy (front & back) of a valid, government-issued (state or federal) Photo ID or a Notarized Statement that verifies his/her identity
- Information required to conduct a search
INFORMATION ABOUT THE INDIVIDUAL/ORGANIZATION TO WHOM THE RECORDS ARE TO BE RELEASED:
- Name
- Phone number
- Email address
- Postal mail address
DFAS will not accept another agency’s request/release/claim form to obtain records. In lieu of a written/typed statement, the individual to whom the records pertain may utilize the Request Pertaining to Military Records (SF-180) to authorize the release of personal records. A copy of the individual’s photo ID or notarized statement, as detailed above, must accompany the signed SF-180.
