Your Benefits and Beneficiary Forms
Permanent and term employees are entitled to benefits. If you are a permanent or term employee, please be sure to visit the benefits information page for details.
Employees are required to use the Employee Benefits Information System (EBIS) to enroll in certain benefits (Health, Life, and Thrift Savings Program). You are eligible to participate in the Federal employee benefits programs once you report to work. The following forms pertain to designating beneficiaries for the various benefit programs. If applicable, please complete these forms and bring them with you on your first day. The beneficiary forms completion checklist is a helpful tool to ensure you have completed all applicable forms.
SF-1152 Unpaid Compensation – Designation of Beneficiary

Total Compensation
