Fort Benjamin Harrison ID Card Office

The Fort Ben Harrison ID Card Office services all Active Duty, Reserve/Retired Military, National Guard, DOW Civilians, and Dependents. We are located on the north side of the Major General Emmett J. Bean Federal Center in Indianapolis, Indiana.

ID Card Eligibility

Proof of Identity is Mandatory. Below is guidance for acceptable identification documents.

  1. Two (2) valid, unexpired forms of identification must be presented (Over the age of 18) to receive any ID card. For a list of authorized proof of identity please see the Department of War (DOW), Lists of Acceptable Documents.
  2. All documents must be originals, certified or notarized copies.
  3. All foreign documents must be originals with an Apostille and translated in English. Please contact the department of Foreign Ministry Affairs in the country of issuance or Google "Apostille certification & country" to obtain guidance.
  4. All court documentation must be state or county certified stating that it has been filed and recorded in the courthouse.
  5. Driver's Licenses must have an actual date of expiration.

Replacing Expired ID Card

Common Access Card (CAC)

The earliest a CAC can be renewed before expiring is 90-days, with the following exceptions:
  • Card is damaged
  • Significant change in appearance
  • Change of status (i.e. promotion, branch of service or contracting agency)

Dependent ID Card

For a Dependent to renew their expiring ID Card, they must provide all of the following items:
  1. Sponsor present
  2. Two forms of valid ID AND/OR
  3. Power of attorney
Family Members ID cards are reissued no earlier than 90 days prior to expiration date

Note:  DD Form 1172 valid for 90 days only

Replacing Lost or Stolen ID Card

DOW Civilians
DOW Civilians need three items:

  1. Memo from your supervisor stating CAC is lost and needs replacement.
  2. Two forms of identification such as: Driver's license (cannot be expired), Social Security Card, or original Birth Certificate.
DOW - DFAS Employees and Contractors (ONLY) need three items:
  1. Two forms of identification such as: Driver's license (cannot be expired), Social Security Card, or original Birth Certificate.
  2. Have supervisor present

Military (All Branches)
For stolen CAC cards, bring:

  1. A police report stating that CAC was stolen.
  2. Two forms of identification such as: Driver's license (cannot be expired), Social Security Card, or original Birth Certificate.
For lost CAC cards, bring:
  1. A Counseling Statement or Unit Letter from your Unit or Commander reporting the CAC was lost.
  2. Two forms of identification such as: Driver's license (cannot be expired), Social Security Card, or original Birth Certificate.

Family Members and Retired Service Members
Family Members need four items:

  1. Two forms of identification such as: Driver's license (cannot be expired), Social Security Card, or original Birth Certificate.
  2. One of the following:
    • Have sponsor present OR
    • General/Specific Power of Attorney (specifying DEERS/ID Cards) OR
    • Original DD Form 1172-2 signed by the sponsor and ID Card Issuing Facility verifying officer

Parking

To park at the Fort Benjamin Harrison ID Card Office, refer below:

  • You can park anywhere in the parts designated as Open Parking (in yellow). 
  • To park in Visitor Parking (marked green), you must report the Make, Model, and License Plate Number to the Security Office (located near the ID Card Office).
  • To use a Handicap parking spot (marked blue), your vehicle must have a handicap placard hanging on the rearview mirror OR your vehicle must have a disability license plate.

ETS and Retirement

How to Obtain a New Identification Card After Separation:

  1. Bring a current government-issued ID card AND another valid form of identification
  2. Bring a copy of DD Form 214 (Member 4 copy)
  3. Bring a copy of your separation/retirement orders

Additional Notes:

  • ID cards are issued on your official retirement date, which is the day after your last active duty service date (End Term of Service) (ETS) or at the end of your terminal leave.
  • If the current ID card is no longer valid, then a second valid form of identification must be another picture ID. The driver's license can not be expired.
  • Retired ID cards can be issued up to 120 days prior to the established retirement date. The 120 days accommodate the combination of the maximum 90 days leave that may be granted, plus 20 days permissive TDY for house hunting.
  • The active duty CAC ID must be surrendered to receive the retired ID Card.
  • Your TRICARE benefits could be affected if you obtain your retiree ID card prior to your actual retirement date. This can also affect your status as retired. This also applies to your dependents. We strongly suggest waiting to obtain your retiree ID Card until the first day you are no longer on active duty.

Secondary Dependency

Secondary (non-primary) dependents can include:

  • Parent, parent-in-law, step-parent, parent by adoption, or any person who stood ‘in-loco parentis’ (in the place of a parent) to you for at least five years prior to your emancipation.
  • Unmarried children ages 21 and 22 who are enrolled in an accredited institution of higher education on a full-time basis.
  • Ward of the court, unmarried, and placed in the permanent legal physical custody of the member, or if not permanent custody for a period of at least 12 consecutive months.
  • Unmarried child over age 21 incapable of self-support because of mental or physical incapacity that occurred while the child was considered a dependent of yours as a member or retired member, or is considered the dependent of a deceased member (while under age 21 or under age 23 and a full-time student).
