Correction of Military Records

If you believe you are the victim of an error or injustice that has impacted your military record, you may be eligible to apply for a Correction of Military Records.

How do I Apply for a Correction of Record?

Applications for Correction of Record are processed by the Branch of Service (BoS) as noted below:

Army

Follow the steps below to submit an application for correction of military record.

1. Prepare

Complete the DD Form 149 – Application for Correction of Military Record. *Please note: You may need to right click to download or save the documents to your computer before you are able to fill them out.*

2. Submit

Submit the completed form and other required documentation to:

Submit the completed form and other required documentation through the Air Force Board for Correction of Military Records website and portal.  However, if you are not able to access this site, you can submit your hardcopy application package by mail to:

If You Have Not Received a Decision:

 

If you are pending a response from Board for Correction of Military Records and want to know the status of your request, contact the Board directly at: If You Have Received Approval From the Board for Correction of Military Records:

If you received approval and want to know the status of your case, submit an AskDFAS ticket.

After submitting your claim via AskDFAS, a ticket number will be emailed to you for reference and updates on your request. Please keep this ticket number until you receive a response.

 Disagreements

If You Disagree With the Board for Correction of Military Records Decision:

Write the board and explain your reasoning for disagreement. Reference the appropriate address under "How Do I Apply for a Correction of Record?" At the top of this page. DFAS can only pay when the board rules in your favor.

If You Disagree With the Amount Received From DFAS:

If you believe DFAS paid you incorrectly for your correction of record, please provide DFAS a written request with a wet signature along with supporting documentation explaining why you are protesting the amount to:
