Public Service Loan Forgiveness

The DFAS HR Benefits Customer Care Center completes the Certification of Employment section of the Public Service Loan Forgiveness (PSLF) form, and returns the form to the requester.
 

DFAS CAN ONLY CERTIFY THE PSLF FORM FOR EMPLOYEES OF THESE AGENCIES: Defense Acquisition University Defense Contract Audit Agency Defense Finance and Accounting Service Defense Information Systems Agency Defense Technical Information Center Department of Defense Inspector General National Defense University/Defense Security Cooperation Agency U.S.  Army Financial Management Command   ALL OTHER requesters must forward the PSLF form to their current [or former] respective Agency Human Resource Office for certification.

Submitting the PSLF Request for Certification to DFAS

I. Complete Sections 1, 2, 3 and 4 of the PSLF form. 

1. FEIN is 31-1575142.
2. Borrower’s signature on page 1 is not required prior to certification.

*DO NOT ELECTRONICALLY SIGN YOUR APPLICATION WHEN SUBMITTING TO DFAS*
Doing so prevents DFAS from including an agency signature​​​

*PLEASE ENSURE YOU USE THE CORRECT RECIPIENT EMAIL ADDRESS*

II. Forward your PSLF form using 1 of the below methods:

1. https://studentaid.gov/pslf/  (preferred)
   1. Recipient Name – DFAS HR Benefits Customer Care Center
   2. Recipient Email - dfas.indianapolis-in.zh.mbx.ssc-cc@mail.mil
2. AskHR ticket [Category: Careers; Subcategory: Public Service Loan Forgiveness (PSLF)]
3. Email dfas.indianapolis-in.zh.mbx.ssc-cc@mail.mil (those who are unable to access AskHR and non-DFAS employees of the agencies listed above).
4. DoD Safe Drop-off (if you have access)  
   1. Recipient Name – DFAS HR Benefits Customer Care Center
   2. Recipient Email - dfas.indianapolis-in.zh.mbx.ssc-cc@mail.mil

*IF YOU SUBMIT THE PSLF FORM VIA DOD SAFE*
 Do NOT encrypt or password protect the form. Doing so prevents DFAS from retrieving it.

Please allow adequate time for processing – typically a week.

The DFAS HR Benefits office is currently closed to the public. Further, we are not presently accepting forms via fax.
