To sign up for, or change direct deposit information fax one of the following to 216-367-3430 (DSN 580-7841) or via e-mail to CCO-216-367-3430@DFAS.MIL , either:

1) A completed DD2762 (available under "Travel Forms" ) to 216-367-3430 (DSN 580-7841) or via e-mail to CCO-216-367-3430@DFAS.MIL ...or

2) Documentation to include the following information:

• The Traveler's name
• The Traveler's SSN
• The Traveler's address
• The routing number
• The account number
• Whether the account is Checking or Savings

*A copy of the orders is also required if the EFT information is new and is not accompanying a claim. Direct deposit is highly encouraged to speed up your payment process.
