 Too often, DFAS hears from surviving spouses who did not receive the retired pay for the portion of the last month the retiree was alive because this money went to someone else whom the member had named at retirement.  In order to prevent this unfortunate situation, it is imperative that an updated beneficiary is on file with DFAS.  The designation of beneficiary form can be accessed at http://www.dod.mil/dfas/retiredpay/beneficiarycard.html

In the event of the death of a retired member or annuitant, DFAS should be contacted as soon as possible.  A photocopy of the certificate of death will be required, however it is not needed for the initial notification of death to be made.

To make it easier for family members in the event of death, retirees and annuitants may want to prepare a file of information for their family’s use.  This file may include any insurance information, allotments, account numbers and agencies with telephone numbers to contact.  Retirees should also let their trusted representatives know where to locate their DD214, separation documents or retirement orders.  Spouses should also know what to expect in terms of benefits and who to contact. 

REMINDER:  Please keep records updated in the event of life changes, such as  marriage, divorce, or death of spouse/beneficiary.
