New Look - Data for Payment of Retired Personnel

The DD Form 2656 “Data for Payment of Retired Personnel” was revised. The date of the revised form is February 2008. To retrieve an electronic version of the form, go to http://www.dtic.mil/whs/directives/infomgt/forms/eforms/dd2656.pdf

The primary changes to the DD2656 are:

1. Clarification of the instructions for who is required to complete the
SBP spousal concurrence.
2. The requirement that all spousal concurrence statements are
notarized.
3. Clarification of SBP base amounts for member who elected the Career
Status Bonus (CSB) and whose retired pay is subject to REDUX.

Use of the new form was required effective May 1, 2008.
