Life Insurance
If you carry Federal Employees Group Life Insurance (FEGLI) Family coverage, you may
make a claim for death benefit
with the FEGLI Office. You can download the required Form
FE-6 Dep here
, or contact your Human Resources Office to request one.

If there are no other family members eligible for this coverage, you should cancel your Option C Family coverage. This does not happen automatically. Contact your Human Resource Office for the proper forms to do this.
Health Insurance
If there are no other family members, you should change your health benefits to a self-only enrollment. You can do this at any time. Contact your Human Resources Office to do so.
