If you experience the death of a spouse, or the death of your child, the information below will provide helpful information and assist you in filing claims and making any applicable benefits related changes during this difficult time. You may also want to consult our Benefits  section for more information.

Life Insurance

If you carry Federal Employees Group Life Insurance (FEGLI) Family coverage, you may make a claim for death benefit with the FEGLI Office. You can download the required Form FE-6 Dep here , or contact your Human Resources Office to request one.

If there are no other family members eligible for this coverage, you should cancel your Option C Family coverage. This does not happen automatically. Contact your Human Resource Office for the proper forms to do this.


Health Insurance

If there are no other family members, you should change your health benefits to a self-only enrollment. You can do this at any time. Contact your Human Resources Office to do so.


Other Considerations...

  • Are your designations of beneficiary forms current? You may want to update your designations of beneficiary for life insurance or for retirement. See your Human Resources Office for details.
  • You may want to change your income tax withholdings. You can do this by using myPay or by contacting your HR/payroll office.
  • Information on Temporary Continuation of Coverage
